Job description
Carle Health’s AMS pharmacist role focuses on direct patient care for anticoagulation management under a collaborative practice agreement. You will implement, monitor, and adjust warfarin and DOAC therapies, with extensive patient education and supervision of AMS staff.
The position requires an Illinois Pharmacist license and a Bachelor’s degree in Pharmacy, working with multidisciplinary teams to optimize therapy, monitor INRs, and coordinate transitions of care within Carle Health facilities
