Job description
Northwell Health's PACE program seeks a Clinical RN Assessment Manager to lead comprehensive assessments for participants, using UAS-NY to determine eligibility and care needs, and to guide personalized service plans.
You will collaborate with intake, enrollment, care delivery teams, and site leadership to ensure accurate documentation, QA, and timely transitions, while adhering to DOH, CMS, and UAS-NY guidelines. Travel across boroughs required.
