Job description
VNS Health seeks a Clinical Assessment RN to complete in-home UAS assessments for diverse populations across NYC. The nurse will evaluate medical, functional, and psychosocial needs, document findings in electronic systems, and collaborate with an interdisciplinary team to develop person-centered care plans. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field while upholding VNS Health’s values of compassion, integrity, and collaboration.
