nJOB SUMMARY AND PURPOSE
nThe Clinical Case Management Team provides clients with the highest levels of acuity and co-morbidities individualized care through access to service linkages, counseling, treatment planning, and various crisis intervention methods in order to prepare individuals for further success. The Clinical Case Manager I provides Intensive Case Management Services to clients who have several psychological co-morbidities that must be managed appropriately for a successful re-entry into society.
nDUTIES AND RESPONSIBILITIES
nUnder supervision of licensed Program Supervisor or Manager, the Clinical Case Manager I:
nnnConducts comprehensive screenings and assessments with individuals (and families as appropriate) to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information, as appropriate
n
nnAssess barriers facing the clients and develop a case plan.
n
nnCoordinate individualized planning with clients to meet short- and long-term needs.
n
nnProvide service linkages and support systems to ensure identified needs are met.
n
nnCoordinates case conferences with clients, consults with service team, monitors progress of clients on case plan, assists clients to meet the agreed upon goals.
n
nnActs as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitates support groups and client participation
n
nnNetwork and communicate with referral agencies and community resources, as well as family members and natural supports
n
nnParticipates and assists with outreach events
n
nnOther tasks as may be required to meet the program goals, and goals of program participants
n
n nESSENTIAL ABILITY ACKNOWLEDGEMENT
nAbility to work and deal with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs, which may include clients with homelessness, mental illness, substance abuse, recently released from incarceration, and other challenges.
nESSENTIAL DUTIES:
nnnServices for this position must be provided on-site. As a result, the position requires incumbents to report to work on-site each day of active employment.
n
nnAssessment: through individual meetings on a regular basis and as part of Case Management team develop a detailed plan of action to meet the client-s needs, set goals and find the necessary resources to meet those goals
n
nnCoaching and Social Support - accompany participants to activities and services
n
nnCoordinate Care and Case Management as part of team, connecting participants to appropriate services
n
nnAdvocacy for participants before other agencies/community settings
n
nnCapacity Building - motivating participant progress
n
nnOutreach and Engagement - Network with service providers, client family members and other natural supports to promote client goals.
n
nnOther duties as required and may be subject to change
n
n nMANDATED REPORTER
nAs service providers to the underprivileged and vulnerable in our community, all employees are considered Mandated Reporters, and must report any instance of physical or sexual abuse to proper authorities.
nQualifications
nREQUIREMENTS:
nnnMust be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications
n
nnValid California driver-s license, acceptable proof of automobile insurance, and/or reliable mode of transportation
n
nnAbility to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients as VOALA serves the neediest of the needy in our programs which may include clients with homelessness, mental-illness, substance abuse, recently released from incarceration and other challenges.
n
n nEDUCATION:
nnnMaster's degree in social work, psychology, or counseling from fully-accredited college or university
n
nnRegistered with the Board of Behavioral Sciences as an Associate Clinical Social Worker (ASW) or Associate Marriage and Family Therapist (AMFT)
n
n nEXPERIENCE:
nnMinimum of 1 year of experience providing human services to a special needs- population
n nPREFERRED QUALIFICATIONS:
nn2 years of complex clinical experience in program(s) serving homelessness, addictions, under-served populations or prison re-entry/diversion
