Job description
Strive Health is seeking a Clinical Lead NP to drive longitudinal, home-based care for patients with CHF, CKD/ESKD, and other chronic conditions. You will work with an interdisciplinary team to deliver personalized, data-driven care beyond traditional 15-minute visits.
You will lead transitions across settings, optimize treatment plans, and participate in discussions around end-of-life care when appropriate.
