Job description
Vynca in California seeks a Clinical Lead Care Manager (CLCM) to coordinate care across providers and empower clients to achieve wellness and independence. This hybrid role requires traveling within the Sacramento County area up to five days weekly and residing within 25 miles of the territory.
CA licensure (LPCC/LCSW/LMFT) and 1+ year in care management are required; English/Spanish bilingual is preferred.
