Job description
Strive Health is seeking a Clinical Lead NP to guide care for patients with chronic kidney disease, heart failure, and related conditions. You will provide longitudinal, home- and telehealth-based care, leveraging analytics and collaborative platforms to reach patients when they need it most.
In this role, you will lead a panel of patients, coordinate with external providers, and participate in shared decision-making.
