Job description
Genesis HCC seeks a Clinical Liaison to coordinate smooth transitions from hospital to post-acute care in our skilled nursing and rehab centers. You will assess clinical appropriateness, review medical records, and collaborate with physicians, discharge planners, and our interdisciplinary team to support safe, timely admissions. The role blends clinical knowledge with relationship-building, educating patients, families, and referral sources about services, coverage, and care plans while promoting Genesis HCC’s patient-centered, recovery-focused model.
