Clinical Liaison -

n· Serve as the vital link between our center and hospital partners
n· Help guide patients and families through the next step in their care journey
n· Work with a supportive, experienced team that is committed to your success
n· Make a meaningful impact while driving admissions with purpose and strategy
nKey Responsibilities
n· Build and maintain strong relationships with medical professionals, hospitals, and community healthcare partners
n· Represent and promote the organization’s mission, philosophy, and services both within hospitals and throughout the community
n· Attend facility meetings, calls, and educational in-services
n· Maintain accurate and up-to-date referral tracking logs
n· Provide timely end-of-day reports to department heads
nQualifications
n· Valid driver’s license required; travel is a key component of this role
n· RN, LPN, or LSW preferred, but not required
n· Ability to manage multiple residents and referrals in a fast-paced environment
