Job description
University Hospitals – Parma Medical Center is seeking a Clinical Liaison to support safe, timely transitions of care and grow referrals across our community. The Clinical Liaison will serve as the key connector between patients, families, referring providers, and the hospital, performing clinical assessments, coordinating admissions, and educating stakeholders on available services. In this mission-driven, patient-centered role, you will collaborate closely with physicians, nursing, case management, and post-acute partners, leveraging UH’s academic resources, advanced technology, and multidisciplinary teams to promote optimal outcomes and excellent patient experience.
