Clinical - Long-Term Services and Supports Care Manager (RN)Immediate need for a talented Clinical - Long-Term Services and Supports Care Manager (RN). This is a 06+months contract opportunity with long-term potential and is located in New Castle County, DE (Hybrid).Key Responsibilities:Evaluate the needs of members and recommend plans for optimal healthcare outcomes.Develop ongoing long-term care plans and identify necessary providers and community resources.Coordinate between members, families, and care providers to ensure accessibility of services.Provide resource support for various needs, including employment and independent living.Monitor care plans and member status, making recommendations based on identified needs.Interact with healthcare providers to ensure comprehensive care delivery.Conduct home visits to assess member needs and collaborate with partners.Educate members and families on healthcare options, procedures, and benefits.Collect and maintain member information to ensure compliance with regulations.Support the case management team in managing increased membership demands.Key Requirements and Technology Experience:Associate's Degree with 4 years of related experience or Bachelor's degree and 2 – 4 years of related experience.Delaware/Compact RN License.Driver's license and a home workspace.Must have: Experience evaluating older adults/disabled level of functioning and medication reconciliation.Nice to have: Case Management Experience, Service Care Manager experience.Pyramid Consulting, Inc. provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
