Job description
The Chinle Service Unit in Chinle, Arizona, is seeking a Nurse (Case Management) to provide advanced outpatient case management and care coordination. You will assess complex conditions, develop care plans, educate patients, coordinate referrals, and support nursing staff to improve continuity of care and access to services.
Qualifications include an accredited nursing degree, active RN licensure, and eligibility for GS-9/GS-11 grade levels.
