Job Summary and Responsibilities
nAs our Pharmacist, you'll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.
nEvery day you will meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities. You'll then authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station, ensuring expert oversight in every process.
nTo excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.
nnnParticipates in the evaluation and management of diseases and health conditions in collaboration with other health care providers
n
nnPrescribe pharmaceutical agents as outline in the approved collaborative drug therapy management agreement including initiating, adjusting, and discontinuing drug therapy
n
nnOrder and interpret drug therapy-related tests
n
nnEnsure patients have access to and receive safe, efficacious and cost-effective medication therapy
n
nnWork collaboratively to resolve medication access and adherence issues
n
nnAddress medication-related issues to support care coordination and transitions of care
n
n nJob Requirements Required
nnnGraduation from an accredited school of pharmacy, upon hire and
n
nnPharmacist: WA, upon hire
n
n nPreferred
nnnDoctor Of Pharmacy, upon hire and
n
nnCompletion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities or
n
nnCompletion of a PGY2 Ambulatory Care Pharmacy residency or
n
nnCompletion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities

n$69.30 - $94.00 /hour
