nJob Description:

nThis position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost-effective pharmaceutical care within the scope of their assigned practice. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.

nThis position will work 7 on 7 off graveyards from 22:00pm - 08:00am.

nEssential Functions

nnnReviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.

nnOversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.

n

nnProvides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.

n

nnCompletes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete. Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.

n

nnSupervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals.

n

nnTakes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns.

n

nnIdentifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedback

nnSupports clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service.

nnnMinimum

nnDoctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).

n

nnLicense to practice pharmacy in the state of practice and dispense controlled substances

n

nnCurrent knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.

n

nnEffective communication, people management, and project management skills.

n

nnImmunization certification if applicable to practice site needs

n

nnPreferred

n

nnPrevious experience working in an integrated healthcare delivery system

n

nnBoard Certification

n nPhysical Requirements

nnnOngoing need for employee to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.

nnFrequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.

n

nnManual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.

nThe hourly range for this position is listed below. Actual hourly rate dependent upon experience.

n$62.89 - $92.44
