Job description
A clinical pharmacy role is available for a Sterile Compounding Pharmacist on a 13-week contract in Dallas, TX. The role features a rotating schedule with one week on the 7:00 AM to 3:30 PM shift followed by one week on the 12:00 PM to 8:00 PM shift.
Key Responsibilities:
Evaluate patients for eligibility and appropriateness of home infusion therapy.
Develop, monitor, and adjust individualized medication regimens using clinical assessments, lab results, and pharmacokinetic data.
Prepare, verify, and dispense sterile medications adhering to USP standards and regulatory requirements.
Provide education to patients, caregivers, and healthcare providers on medication therapies and infusion techniques.
Collaborate closely with healthcare teams to ensure continuity of care and monitor patient outcomes.
Manage infusion pumps and related equipment with focus on quality assurance and safety.
Maintain accurate documentation, support regulatory compliance, and promote medication safety initiatives.
Qualifications & Desired Experience: Doctor of Pharmacy (PharmD) or graduate from an accredited School of Pharmacy.
Active, unrestricted Texas Pharmacist license.
At least one of the following: • Minimum 1 year of sterile IV compounding experience, • Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM), • Completion of an ASHP-accredited Pharmacy Residency, • Completion of an Advanced Pharmacy Practice Experience (APPE), • Equivalent pharmacy experience.
Strong clinical knowledge of home infusion pharmacy practices.
Expertise in interpreting laboratory values and applying pharmacokinetic principles.
Excellent communication and patient counseling skills.
Location: Dallas, Texas
Schedule: Rotating weekly shifts:
• Week 1: 7:00 AM - 3:30 PM
• Week 2: 12:00 PM - 8:00 PM
