Job description
UChicago Medicine is seeking a Clinical Pharmacist, Registry, on a part-time basis (under 24 hours per week) for the Meds2Beds Pharmacy at the Hyde Park campus. The role focuses on safe, effective, timely dispensing at hospital discharge and patient-centered drug therapy in collaboration with a multidisciplinary team.
Responsibilities include discharge counseling, medication reconciliation, and education to physicians, nurses, and patients.
