Job description
CornerStone Medical is seeking a Pharmacist for a 13 week assignment in the Fort Worth area. $95/hr Mon-Fri, 8a-5p Onsite seeing 14 patients per day as a mid-level provider.
Job Duties:
Provides clinical leadership for pharmaceutical care in area(s) of responsibility. Serve as a resource for outpatient and inpatient clinicians for HIV and/or Hepatitis pharmacotherapy. Works with the Inpatient Infectious Diseases Clinical Pharmacist to transition HIV patients from the hospital setting to the HIV Clinic on appropriate therapy. Develops and optimizes pharmacotherapeutic regimens through collaborative practice agreements. Performs drug utilization reviews, monitors for drug interactions and potential adverse events. Works to minimize polypharmacy in our patients and develops patient medication action plans with self- management goals. Preceptor for pharmacy students or residents, as well as participating in didactic and experiential education. Participates as a key component to the core multidisciplinary team involved in patient care in the Medical Home.
Qualifications REQUIRED:
BCIDP or AAHIVP board certification PGY2 HIV/HCV residency or PGY2 ID with focus on HIV/HCV residency required BLS 5 years of direct HIV patient care that demonstrates medication management.
Preceptor with State Board of Pharmacy Pharmacy license from Texas State Board of Pharmacy
Must pass criminal background check, drug test and have current immunizations as required by the hospital
