Job description
LCMC Health seeks a Clinical Pharmacist to collaborate with physicians and care teams to optimize patient outcomes while controlling costs. The role delivers therapeutic assessments, disease management, and contributes to formulary initiatives across assigned practices.
You will lead clinical pharmacy services, educate healthcare professionals, and participate in protocol development, quality improvement, and regulatory compliance, including ACHC/URAC accreditation readiness and 340B activities.
