Job description
ChristianaCare seeks a Clinical Pharmacist to deliver evidence-based medication management in a collaborative, patient-centered environment. In this role, you will optimize pharmacotherapy, perform order verification and medication reconciliation, monitor for safety and efficacy, and participate in antimicrobial stewardship and formulary initiatives. You will educate patients and care teams, support quality and safety programs, and engage in continuous learning within a diverse, innovative, community-focused health system committed to excellence and equity.
