Pharmacy - Clinical Pharmacist 8 days ago Rochester, NY
A Home Infusion Staff Pharmacist position is open in Rochester, NY, offering a Monday through Thursday schedule from 8:00 AM to 5:00 PM. This direct hire role focuses on clinical pharmacy services within a home infusion setting, utilizing the CareTend system.
Responsibilities include reviewing infusion orders and patient therapies, participating in clinical rounds, performing sterile compounding and TPN reviews, and maintaining patient records and care plans. The pharmacist will ensure compliance with USP standards and cleanroom protocols, provide training and support to staff, and contribute to operational improvements.
Qualifications:
Current licensure as a Registered Pharmacist in New York State
Minimum of two years recent acute care experience or at least one year of recent home infusion therapy experience (preferred)
Strong knowledge and experience in intravenous admixture services and intravenous therapy
Ability to evaluate and monitor drug therapies in collaboration with healthcare professionals, considering the comprehensive needs of home care patients
