Clinical Pharmacist - Inpatient Hospital
Location: Zanesville, OH
Job Type: Contract
Contract: 13 Weeks
Pay Rate: $ 2,200 To $ 2,492 Per week
Schedule: Day/Evening Shifts

About the Role
We are seeking an experienced Clinical Pharmacist - Inpatient Hospital for a 13-week contract in Zanesville, Ohio. The Pharmacist will provide inpatient pharmacy services while supporting safe and effective medication management across the hospital setting.
Candidates must have hands-on experience with pharmacokinetic dosing, including vancomycin, and warfarin management . Experience with Epic Willow Inpatient is preferred but not required.
Responsibilities
Provide comprehensive inpatient pharmacy services and support safe, accurate, and appropriate medication therapy.
Review medication orders and apply clinical pharmacy knowledge to optimize pharmacotherapy and patient safety.
Perform pharmacokinetic dosing and monitoring, including vancomycin dosing and therapeutic management.
Provide appropriate warfarin dosing and anticoagulation management based on clinical parameters and established protocols.
Collaborate with physicians, nurses, and other healthcare professionals while maintaining accurate pharmacy documentation and supporting continuity of care.
Requirements
Minimum 1 Year of Recent Inpatient Hospital Pharmacy Experience Required.
Current, Active Ohio Pharmacist License in Good Standing Required.
Experience With Pharmacokinetic Dosing, Including Vancomycin, Required.
Experience With Warfarin Management Required; Epic Willow Inpatient Experience Preferred.
Ability to Work Varied Day/Evening Shifts, Including Weekends and Required Holiday Coverage.
This contract opportunity is suited for an experienced hospital Pharmacist with strong inpatient clinical pharmacy, pharmacokinetic dosing, and anticoagulation management experience.
