Job description
Encompass Health seeks a Clinical Pharmacist to support our inpatient rehabilitation hospital within the Healthcare & Medical Services department. You will provide evidence-based medication therapy management, verify and dispense orders, and collaborate closely with physicians, nurses, and the interdisciplinary team to ensure safe, effective pharmacotherapy. Responsibilities include medication reconciliation, patient counseling, drug interaction and ADE monitoring, and participation in clinical rounds and quality initiatives.
