Job description
Clinical Pharmacist – Per Diem, Variable Schedule
Position Summary: Provide pharmaceutical services to SPH inpatients and outpatients and to satellite clinics.
Responsibilities
Order Entry: Enter medication orders into the Meditech computer system, init orders, review for appropriateness, perform dose calculations and adjustments for renal/hepatic function, conduct therapeutic interchanges, document interventions.
Resolve drug therapy problems: intervene with physicians or team members regarding drug-drug interactions, allergies, duplication, and clarify incomplete or inaccurate orders.
Patient Care:Review medication profiles and intervene if a problem is identified.
Provide drug information and pharmacokinetic consults.
Manage TPN orders: review labs, document and perform necessary interventions.
Perform therapeutic drug monitoring on appropriate drugs (e.g., warfarin, anticonvulsants, digoxin).
Review labs and microbiology cultures and intervene with physicians when needed.
Patient education: teach first dose, diabetes, warfarin reach, discharge counseling.
Quality Control and Preparation: Guide final quality checks on drug selection, preparation, packaging, labeling, unit dose cart; supervise preparation of sterile products, injections, IV admixtures, TPN.
Training and Supervision: Assist with training of new department employees, provide drug information and pharmacotherapy recommendations to other professionals, direct supervise pharmacy technicians (1-3) and one intern/student.
Documentation: Maintain drug storage, record keeping, utilization data; document adverse drug reactions; report to committees.
Requirements
Education: Required – PharmD degree or Bachelor’s degree in Pharmacy with equivalent experience.
Experience: Required – 1-2 years of hospital experience as a Pharmacist.
Licenses: Required – State of Wisconsin license as a Pharmacist.
Certifications: Required – Basic Life Support (BLS) within 3 months of hire and Advanced Cardia Life Support (ACLS) within 6 months of hire.
