Job description
Axil Health in North Carolina seeks a Clinical Pharmacist to lead Chronic Care Management, Transition Care Management and Remote Patient Monitoring within a Collaborative Practice Agreement across independent clinics. You will build patient panels and drive clinical outcomes in a dynamic team.
You will enroll patients, interpret RPM data, collaborate on medication management with providers, and precept PGY-1 residents while developing individualized care plans and therapy changes as needed.
