Job description
ChenMed is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics under a collaborative practice agreement. You will conduct comprehensive medication reviews, manage chronic diseases, and coordinate transitions of care to elevate patient outcomes and CMS/HEDIS metrics.
Responsibilities include patient education, interpretation of lab results, and collaboration with physicians and care teams.
