Job description
We're Hiring: Clinical Pharmacist (Per Diem) | Sauk Prairie Healthcare
Looking for a flexible opportunity to make a meaningful impact on patient care? We're seeking a Clinical Pharmacist (Per Diem) to join our team and provide pharmaceutical services for inpatient, outpatient, and clinic settings.
What you'll do:
Review and verify medication orders for safety and appropriateness
Provide clinical interventions and pharmacotherapy recommendations
Perform therapeutic drug monitoring and pharmacokinetic consults
Manage TPN orders and sterile product preparation oversight
Educate patients and healthcare team members on medication therapies
Participate in Code Blue response and interdisciplinary care teams
Qualifications:
PharmD or Bachelor's Degree in Pharmacy with equivalent experience
1-2 years of hospital pharmacy experience preferred
BLS within 3 months of hire and ACLS within 6 months
Schedule:
Per Diem (minimum 3 shifts/month, based on department needs)
