Job description
University Hospitals seeks a Clinical Pharmacy Specialist in Primary Care for Lake County/Mentor. The role includes rounding with physicians, medication therapy management, staff education, and participating in formulary and policy development.
You will precept residents and students, contribute to safety initiatives, document clinical activities, and collaborate with local and system committees. Requires PharmD, ASHP residency preferred, Ohio license, and strong communication skills.
