Job description
Hartford HealthCare Corp. is seeking a Clinical Resource Management Nurse to join the Care Management team.
The role focuses on timely care assessments at entry into the health system, ensuring appropriate patient class determination based on clinical needs, with autonomy to manage a high volume of complex cases across HHC hospitals. The nurse will apply criteria, document findings, and partner with ED/Admitting providers to ensure appropriate care levels.
