Job description
The Clinical Social Worker responsibilities include:
Provides mental health clinical services to children and adolescents.
Promoting a safe, cooperative and professional health care environment to ensure optimum patient care.
Interviews and evaluates patients.
Gathers, analyzes and evaluates data collected relating to the patient's biopsychosocial and family problems, current mental status and individual/family functioning.
Completes intake assessment, as requested by program Director.
Completes diagnosis evaluations and develops treatment plans, acting collaboratively with the patient and the treatment team in order to define appropriate goals, outcome and time frame of treatment.
Carries a caseload at an acceptable level as defined in the Program's service commitments and productivity goals.
Follow up with patients who fail to appear for appointments and documents the results of that follow up.
Maintains a complete and accurate medical record for each client in accordance with Medical Records Policy and Procedure.
Maintains networking and referral contacts with other mental health/social services providers.
Attends all staff meetings, case conferences and Center-wide meetings as required.
Makes presentations at case conferences as assigned.
Participates on appropriate committees and handles special projects as assigned.
Education and License required: LMSW, LCSW, or PHD in Psychology with current license.
Experience/Skills Required: Ability to maintain accurate records and interpret information.
Ability to integrate psychological theory, research and practice.
Excellent communication skills.
Ability to organize and prioritize multiple tasks effectively.
Ability to work professionally and cooperatively with a multidisciplinary team approach.
Ability to work under pressure
Ability to speak Spanish strongly preferred.
