Job description
LifeLong Medical Care in Oakland, CA seeks a licensed clinician (LCSW/LPCC/AMFT/APCC/ASW) to join our East Oakland Health Center team. You will deliver psychosocial assessments, diagnoses, treatment planning and short-term counseling within an interdisciplinary primary care setting.
You will provide case management, crisis intervention, and follow-up planning, while maintaining records and supporting quality improvement initiatives. A strong commitment to diverse populations is essential.
