nnHaven Hospice is looking for a compassionate and detail-oriented Hospice Clinical Specialist in Jacksonville, FL to support the delivery of high-quality hospice care through expert clinical oversight, documentation management, and workflow coordination. This vital role ensures patients receive timely , compliant, and well-coordinated care by managing clinical processes from start of care through discharge.

nResponsibilities
nAs a Hospice Clinical Specialist , You will:
nnnProvide clinical oversight for nursing management, documentation, and patient care planning
n
nnProcess workflow following the completion of Start of Care visits to ensure continuity of care
n
nnParticipate in Interdisciplinary Group (IDG) meetings to coordinate and review patient care plans
n
nnReview and adjust visit schedules for Start of Care (SOC) and recertification visits as needed
n
nnMaintain and manage clinical orders within the HCHB (Homecare Homebase) system
n
nnAssist in managing admissions, discharges, and deaths with proper documentation
n
nnEnsure timely and compliant documentation per agency policies and regulatory requirements
n
nnSupport data collection efforts for compliance monitoring and quality assurance initiatives
n
nnCommunicate effectively with referral sources to facilitate smooth patient transitions into care
n
nnContribute to staff development and nursing policy implementation as assigned
n
n nQualifications
nnnAssociate degree in Nursing with current license
n
nnThree years practice as a Registered Nurse
n
nnPrevious Hospice experience preferred
n
nnCurrent Nursing License
n
nnKnowledge of the Hospice Philosophy of Care
n
nnAbility to use Microsoft Word, Excel, Outlook, and other software programs
n
