Hartford Healthcare seeks a Clinical Team Manager to lead a home health nursing team focused on high-quality, compassionate patient care. In this role, you will supervise RNs and clinical staff, oversee OASIS documentation, and ensure compliance with regulatory and quality standards. You will coordinate patient care plans, monitor clinical outcomes, and provide coaching, mentoring, and performance feedback to staff.
