Job description
CNA – Certified Home Health Aide - Chicago/SkokieThe Home Health Aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The Home Health Aide is responsible for observing patients, reporting these observations, and documenting observations and care performed.The Home Health Aide will be assigned in a manner that promotes quality, continuity, and safety of patient care.Responsibilities of the Home Health Aide include, but are not limited to:Assist clients with personal care activities and provide companionship tasks in accordance with the care plan.Provide assistance with daily activities such as bathing, dressing, and grooming.Assist with mobility and transfers.Monitor vital signs and report any changes to the healthcare team.Maintain accurate medical documentation and records.Perform simple procedures as an extension of nursing services as ordered, with appropriate completed/demonstrated skills competency.Meet the safety needs of patients and use equipment safely and properly, including footstools, side rails, etc.Participate as a member of the interdisciplinary team.Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.The above statements are intended to be a representative summary of the major duties and responsibilities performed by employees in this position. Employees may be asked to perform other job-related tasks as needed.Requirements:Previous experience as a Home Health Aide with current HHA certification.Knowledge of basic medical terminology and procedures.Familiarity with medical documentation and record keeping.Ability to follow care plans and instructions from healthcare professionals.Excellent verbal and written communication skills.Compassionate and patient demeanor.Current CPR certification.Familiarity with medical software and equipment.Ability to build rapport with patients.Ability to thrive in a fast-paced environment.WellSky EMR knowledge preferred, but not required.Job Limitations: The Home Health Aide will not function in any manner that is considered the practice of nursing under the State's Nurse Practice Act. Specifically, the Home Health Aide will not administer medications, take physician's orders, or perform procedures requiring the training, knowledge, and skill of a nurse, such as sterile techniques.
