nSummary of Position: Under the direction and supervision of a licensed nurse, performs basic nursing care procedures.
nEssential Duties and Responsibilities:
nnn Performs basic nursing care procedures under the direction and supervision of a licensed nurse. Follow treatment plan that was designed to meet the appropriate age, needs and abilities of each resident/patient including assisting with meals and nourishment’s, transferring using proper body mechanics and appropriate assistive devices, ambulation, bathing, toileting and dressing and grooming. nn
nn Observes and communicates pertinent information about the adult/older adult resident/patient’s condition to the appropriate facility staff. nn
nn Takes and records pulse, respirations, blood pressures and weight as needed. nn
nn Documents care and observations on work sheets and EMR accurately and in a timely manner. nn
nn Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers. nn
nn Adheres to infection control practices. nn
nn Supports the psychosocial needs of the residents as per the plan of care and profile. nn
nn Participates in educational and inservice meeting to update knowledge of techniques, guidelines and regulations. Must attend all mandatory programs. Meets basic requirement for 12 hours of continuing education per year. nn
nn Uses equipment in an appropriate and safe manner. Adheres to standards of the Diligent Minimal Lift Program. nn
nn Follows facility policies and procedures. nn
nn P erforms other duties as required. nn
n nMinimum Qualifications:
nEducation: High School diploma or GED preferred
nExperience: 6 months – 1 year experience as a CNA preferred
nCertificates, Licenses, Registrations:
nState of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
nKey Competencies:
nAbility to interact professionally with coworkers.
nPrioritize tasks.
nFunction as a team member.
nAbility to speak, read English in order to communicate with patients, residents and clinical staff.
nProvides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.
