We're seeking the right candidate for the position of CNA Household Supervisor at Lakeland Community Care Center . In this vital role, you'll ensure that our residents receive the highest level of care while collaborating with a dedicated team, providing exceptional clinical leadership and support.
What You Bring to the Table:
Essential Qualifications:
This role oversees the social function of the household, including activities. This position serves as a mentor/leader and partners with the Clinical Coordinator in the coordination of care of the residents within the household.
Develops positive resident relationships, exploring approaches to aging and supporting families.
Addresses and documents any special psychological concerns (death in family, health decline etc.).
Maintains individualized psychosocial assessment.
Evaluates personal and emotional concerns.
Assists with the completion of an individualized activity assessment.
Maintains household activity calendar and coordinates activities and ensure resident appointments are kept.
Requirements
The Ideal Candidate: Valid CNA/LPN Licensure
One (1) year experience in long term care, as a CNA/ LPN.
Must have critical thinking skills and decision-making ability.
Must have excellent leadership skills and abilities.
Excellent communication skills to effectively communicate with residents, families, and staff.
