CNAReports To - Community Clinical Team LeaderStatus - Non Exempt - HourlyFT, PT, PRN - Full timeShift - 40 hours per week - M-F 8A -4:30PLocation - Buncombe County and surrounding areasPosition Summary:Provides excellent, professional day-to-day hands-on personal care to individuals with chronic, serious, or life-limiting conditions. CNAs must exhibit competencies required by law and abide by Four Seasons policies in providing personal care to patients, as directed by the written plan of care.Essential Duties & Responsibilities:Practice within the scope of license, certification or registration, where applicable, and as required by law and regulations as delegated to and directed by the registered nurse:Participates in the coordination of the patient's hospice care in accordance with professional standards of practiceCollects data for ongoing interdisciplinary comprehensive assessmentsParticipates in the development and evaluation of the plan of care by reporting observationsContributes to patient and family counseling and educationPerform duties in compliance with Medicare Condition of ParticipationPractice safety and infection control methodsProvision of hands-on personal careWorks to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competenciesKnowledge, Skills & Abilities:Must be willing and able to care for patients with infectious disease processes unless approved accommodationsKnowledge of proper body mechanicsExcellent organization skillsExcellent interpersonal skillsExcellent written and oral communication skillsAbility to work under pressure to meet deadlinesAbility to take initiative and utilize innovative techniques and ingenuityAbility to interact with various departments throughout the organizationAbility to work flexible hours, if neededAbility to participate as a team player to coordinate projectsAbility to read, interpret and apply laws, rules and regulationsAbility to plan, prioritize and coordinate multiple projectsMinimum Qualifications:Education:High School Diploma/GEDExperience:No more than a 24 month lapse in providing clinical care services upon the completion of a State-Accredited Certified Nursing Assistant course0 – 1 year of clinical experience.Licensures & Certifications: (if applicable)Valid NC CNA certificateValid driver's license and proof of automobile insuranceSummary of Physical Requirements:Walking, standing, sitting, bending, reaching, twisting, crouching/squatting, kneelingFlexion/Extension of arms/wristHand dexterity to include gripping, pushing/pulling and writingLifting/carrying of 100 lbs. oftenIntent & Function:All descriptions have been reviewed to ensure that only essential functions and basic duties have been included. Hospice Aides will attend a minimum of 16 hours (includes state required 12 hours) of in-service training yearly by selecting acceptable learning activities by recognized providers of continuing education Hospice Aides must exhibit competency in the observance of Universal Precautions by applying principles of infection control by:Knowing standard precautionsWashing hands before and after patient careWearing gloves and other PPE as necessaryIn communicating any changes in patient condition to the RN case manager, Hospice Aides must exhibit competency in:Maintaining chart documentation of care provided in accordance with the written plan of care.Maintaining confidentiality and privacy of any PHI per HIPAA requirementsCommunicating in a professional manner to internal/external customersExhibiting competency in using software systems such as Outlook and current EMR, to record visits or treatment and to communicateIn maintaining general order, safety and cleanliness of the patient area, Hospice Aides need to be competent in applying safety principles by:Disposing of biohazard materials correctlyBeing acutely aware and knowledgeable of personal and environmental safetyDemonstrating proper transfer and lift techniques using good body mechanic techniquesCleaning and storing DMEs and assisting in storing & tracking patient care products if applicableIn providing excellent patient care, Hospice Aides need to exhibit competency in treatments including:Administering personal careAssistance in ambulation or exercisesAssisting with ADLProviding post-mortem careIn facilitating excellent patient care, Hospice Aides need to exhibit competency in using equipment properly by demonstrating:Use of whirlpool tubAdjusting air mattressesUsing mechanical lift/weights, if applicableUse of glucometerExhibiting knowledge of professional boundaries by:Being able to describe roles and relationships boundaries as a Hospice AideDescribing inappropriate and appropriate behaviors in any given patient situationOther duties as assigned by registered nurse, clinical team leader, EH Manager or Regional Director of Clinical Services at Elizabeth House
