Schuyler Ridge Sub-Acute Rehab Team Position
Join Schuyler Ridge's Sub-Acute Rehabilitation Team Empowering Recovery, One Step at a Time At Schuyler Ridge, our Sub-Acute Rehabilitation Unit is dedicated to helping patients regain strength, independence, and confidence after hospitalization. Whether recovering from surgery, illness, or injury, our team provides compassionate, goal-oriented care in a supportive environment. What You'll Do:
Deliver short-term, high-acuity care focused on rehabilitation and discharge readiness
Collaborate with physical, occupational, and speech therapists to support patient goals
Monitor progress and adjust care plans to promote optimal recovery
Educate patients and families on post-discharge care and wellness

Schuyler Ridge Residential Healthcare, part of St. Peter's Health Partners, is a 120-bed skilled nursing facility offering long-term care, short-term rehabilitation, and specialized memory care services

Your Role:
As a CNA your responsibilities include:
Assisting residents with daily living activities (bathing, dressing, eating, mobility)
Providing compassionate, person-centered care
Documenting care in the EMR system
Supporting a safe and comfortable environment
Collaborating with nurses and interdisciplinary teams
Qualifications:
Current CNA certification in New York State
Passion for working with seniors and memory care residents
Strong communication and teamwork skills
Previous long-term care experience preferred (not required)
Location: Schuyler Ridge Residential Healthcare, Clifton Park, NY Employment Type: Full-Time & Part-Time Shifts Available: Day, Evening, Night Pay Range: $17.55 – $22.93/hour (based on experience)
