Job description
BRC is seeking a field-based case manager for its Metropolitan Apartment Program in New York City. You will handle an assigned caseload within a residential treatment model, offering supportive counseling, service planning, and linkages to medical and behavioral health services.
This role emphasizes harm reduction and collaboration with an interdisciplinary team. The position is full-time (35 hours/week) with after-hours on-call duties, based in clients' apartments in Manhattan/Bronx.
