Job description
The Case Management (CM) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. The CM RN coordinates care with multiple providers across all levels and sites of care and leads discharge planning for inpatient and outpatient settings.
This role emphasizes collaboration with veterans, families, and caregivers to meet complex health care needs, and includes involvement in education, competency development, and outcomes evaluation.
