Job description

Optum AZ is seeking a Primary Care Neighborhood Physician to join our team in Phoenix, AZ. You’ll be part of the Optum Care Delivery team committed to improving health outcomes through collaborative, patient-centered care.

This role emphasizes Medicare Advantage care, 60–90 day post-discharge follow‑up, and a team-based approach with clinic, virtual, and possible home visits. Monday–Friday, 8am–5pm, and relocation assistance for eligible candidates.
