Job description
Northern Counties Health Care, Inc. is seeking a Health Center Nurse (LPN/RN) to join our primary care team. You will deliver compassionate, patient-centered care and collaborate with providers to support diverse populations across all ages.
Responsibilities include patient assessments, care coordination, medication administration, health education and EMR documentation. CPR/BLS, Vermont licensure, and clinical excellence are required.
