Job description

Work Arrangement:

nnnMonday through Friday, 8:30 AM to 5:00 PM

n

nn4 days must be worked on-site at our DC office located at 1201 Maine SW. The 5th day can be worked remotely from home.

n

n nResponsibilities will include moderate or low risk member case load.

nMember Outreach:

nnnEstablishes connection and acts as the primary ACFC contact to a targeted member population, including some high-risk members, through home visits and outreach calls.

n

nnIdentifies SDOH needs and addresses them appropriately.

n

nnExercises judgment and discretion in coordinating care by handling basic care follow-up tasks and collaborating with AmeriHealth Caritas care management teams, community-based health care providers, and other organizations.

n

nnInforms members of the required actions to fulfill HEDIS, EPSDT, and Plan requirements and other referred outreach through phone calls or home visits and encourages them to comply.

n

nnAssists in addressing members’ barriers to care and encourages/fosters relationships with their medical/dental providers (medical/dental homes).

n

n nCommunity Education:

nnnDevelops and maintains relationships with new and established community organizations via phone, mail, e-mail, or personal visits.

n

nnCollaborates with internal departments and external agencies to obtain data that is pertinent to the overall success of health education/promotion programs.

n

nnProvides education to members and member representatives regarding available benefits.

n

n nEducation/Experience:

nnnAssociate’s Degree in Health Care, Human Services, or a related field or equivalent work experience required.

n

nnMinimum of 1 (one) year of health care in an office/clinic/hospital setting or human services experience.

n

nnCommunity-based care experience desired

n

nnAbility to travel up to 50% of the time to meet with members in assigned communities within DC

n

nnKnowledge and understanding of medical terminology and disease processes.

n

nnAbility to effectively communicate preventive health and health management philosophies to members and agencies.

n

nnComprehensive knowledge of Medicaid requirements and plan, policies, and procedures.

n

nnKnowledge of managed care.

nThe targeted hiring range for this role is expected to be between 64,300 and 87,700. The range displayed in this job posting reflects the minimum and maximum for new hire salaries for the position in the Washington D.C. area. Within the range, individual pay is determined by additional factors, including, without limitation, job-related skills, experience, and relevant education, certifications, or training.
