Job description
Hartford Healthcare is seeking a Neighborhood Health professional to deliver compassionate, community-based care across our Connecticut service area. In this role, you’ll provide primary and preventive health services, conduct assessments and screenings, and support chronic disease management for diverse patients. You’ll collaborate closely with an interdisciplinary team, leverage advanced technology and EHR systems, and participate in outreach programs that promote wellness, access, and health equity in local neighborhoods while growing your skills in a supportive, inclusive environment.
