What You'll Do
As a Community Inclusion Specialist , you will:
Support adults with developmental disabilities through community-based activities, outings, and individualized programming
Implement Individual Support Plans (ISPs) and person-centered goals
Facilitate meaningful recreational, educational, vocational, and social activities
Promote independence, self-advocacy, and active participation in the community
Provide transportation to community activities, events, and appointments
Complete required documentation, progress notes, and activity records
Manage outing budgets and activity-related expenses as needed
Ensure participant health, safety, and well-being, including providing basic first aid when necessary

Qualifications Required
High School Diploma or GED equivalent
Minimum of three (3) years of experience supporting adults with developmental disabilities
Valid driver's license and reliable personal vehicle
Basic computer proficiency
Strong communication and interpersonal skills
Ability to lift up to 50 pounds
Ability and willingness to safely transport individuals to appointments, activities, and community-based destinations using a personal vehicle.
Preferred
Bachelor's Degree in Human Services or a related field
Experience facilitating community-based programming and activities
Knowledge of person-centered planning and community inclusion practices
Schedule
Full-Time
Community-Based Position
Flexible scheduling based on participant needs and program activities
Some evening and weekend availability may be required
Physical Requirements
Ability to lift up to 50 pounds
Ability to stand, walk, bend, and accompany individuals in community settings
Ability to safely transport participants using a personal vehicle
Ability to perform essential job functions with or without reasonable accommodation
