Job description
Sheppard Pratt Careers seeks a Community Integration Specialist/Navigator, Residential Transition. This on-site, full-time role offers $20.95/hour with a day shift, in-person duties, and a focus on helping individuals transition from state psychiatric hospitals.
You will coordinate assessments, integration activities, planning, linkage, and advocacy, while maintaining documentation and supporting crisis stabilization, medication monitoring, and transportation needs.
