n$18.85 - 23.00/hour, based on applicable years of experience

n nJob Summary:
nThe Children’s Community Provider provides direct services to children and families in their community, including the CFTSS and HCBS programs. They will foster the highest possible quality of life and personal achievement in accordance with the individual’s person-centered plan.
nEssential Job Duties:
nA Children’s Community Provider is responsible for providing support services to youth with a range of emotional, behavioral and social difficulties.
nnnThey will encourage growth and recognize the autonomy of each individual.
n
nnIt is recognized that the individual being supported drives the work and focus of this position. It is the individual’s home and their life choices.
n
nnAttend paid new worker training and ensure regulatory training standards are met
n
nnProviding required face to face services in the community or home, requires use of personal vehicle.
n
nnAttend team meetings, supervision sessions, staff meetings as scheduled
n
nnForm and maintain relationships with youth and caregivers
n
nnWork with youth to achieve goals consistent with their service plans
n
nnWritten communication skills for documentation, including progress notes, correspondence notes, service plans, and safety plans which show progress toward individual goals, observations and interactions.
n
nnDemonstrate competency with navigating electronic health record (EHR) for documentation of services rendered.
n
nnReview utilization and service provision with supervisor
n
nnCoordinate and collaborate with Care Managers to identify service needs and develop service plans
n
nnEnsure appropriate chain of command is utilized.
n
nnComply with all federal and state regulations and agency policies pertaining to HIPAA, false claims, whistleblower policies and all others.
n
nnAny other duty as assigned by your supervisor

n nMinimum Education/Experience Required:
nnHigh School education with 2 years’ experience in Mental Health, Substance Abuse Disorder, or Foster Care OR Bachelor's Degree in Human Services
n nPhysical Requirements
nnnWork required is one on one or in a group, in the community or in the home where the child lives, supporting the child in various community venues; attending meetings and trainings in administrative buildings as required.

nnThe frequent physical demands of the position are repetitive motions, and eye/hand/foot coordination.
