Community Relations Liaison - Home Health / Senior Care
Location: Sarasota, FL - Office & Field-Based
Compensation: $100,000-$120,000 annually
Job Type: Full-Time
Clinical Background: Registered Nurse (RN) Strongly Preferred
Focus: Community Relations | Healthcare Outreach | Referral Partnerships | Senior Care
Schedule: Occasional Evening & Weekend Community Events
About the Opportunity
Our client is seeking a Community Relations Liaison who combines healthcare knowledge with exceptional communication, relationship-building, and community outreach skills.
This role is ideal for an outgoing, compassionate, and polished healthcare professional who enjoys educating others, developing referral partnerships, and serving as a trusted resource for seniors and their families .
You'll represent the organization with physicians, hospitals, senior living communities, elder care professionals, families, and other community partners while helping identify individuals who could benefit from an innovative approach to senior care.
This is not a traditional sales role. Success comes from building genuine relationships, understanding healthcare needs, educating referral partners, and becoming a trusted advocate within the senior care community.
What You'll Do
• Develop and maintain relationships with physicians, medical practices, hospitals, rehabilitation facilities, senior living communities, elder law attorneys, and other healthcare professionals
• Educate community partners on the organization's RN Care Management services and proactive approach to aging
• Meet with families and healthcare professionals to understand care needs and explain available solutions
• Identify seniors and families who may benefit from additional support, monitoring, care coordination, or other services
• Conduct community outreach, presentations, networking activities, and marketing initiatives
• Attend community events, including occasional evening and weekend events as needed
• Create and publish blogs and other marketing content to support community education and outreach
• Serve as a knowledgeable healthcare resource and trusted representative of the organization
• Collaborate with clinical teams to ensure clear communication and appropriate care recommendations
• Maintain consistent follow-up with referral sources and community partners
• Track relationship development activities and referral opportunities
• Build long-term partnerships throughout the Sarasota healthcare and senior care community
What You'll Bring
• Registered Nurse (RN) strongly preferred
• LPN, healthcare professional, or experienced senior care professional backgrounds will also be considered
• Strong understanding of senior care, chronic conditions, and healthcare navigation
• Excellent verbal and written communication skills
• Strong interpersonal and relationship-building abilities
• Professional, polished, and confident when presenting to physicians, executives, families, and community groups
• Warm and empathetic approach with the ability to establish trust quickly
• Strong organizational skills with the ability to manage multiple relationships and priorities
• Self-motivated with the ability to independently manage field-based responsibilities
• Comfortable networking, attending community events, and developing professional partnerships
