Job description
Neighborhood House seeks a Resource Navigator to provide culturally competent outreach, face-to-face case management, and care coordination for individuals and families with complex needs. You will connect clients to health care, housing, food, behavioral health, and other services.
This hybrid role requires engagement with diverse communities, client-centered assessments, and strong partnerships with local service providers to improve well-being and self-sufficiency.
