nDescription
nCommunity Service Provider – Youth Peer
nBe an individual 18 to 30 years old who has self-identified as a person who has first-hand experience with, emotional (mental health), behavioral challenges, and/or co-occurring disorders
nAs a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing peer support, caregiver support and advocacy. You will report to a Community Service Provider Supervisor, and work onsite within the community, as travel is required and documentation may be completed remotely.
nResponsibilities:
nnnProvide services within a Family-driven Care framework, including respecting family choice and voice, believing in each family’s potential to learn and grow, and utilizing strategies that empower families
n
nnWork with family members to identify goals and objectives of treatment
n
nnProvide services based on the needs as identified in the Treatment Plan
n
nnCoordinate with the child and caregiver on the scheduling and provision of services
n
nnParticipate in relevant treatment meetings and attend all required trainings
n
nnComplete all documentation
n
n nRequirements and Education:
nnnHigh School diploma or GED
n
nnValid NYS Driver's License required
n
nnBe an individual 18 to 30 years old who has self-identified as a person who has first-hand experience with, emotional (mental health), behavioral challenges, and/or co-occurring disorders
n
nnBe able to use lived experience with a disability, mental illness, and involvement with juvenile justice, special education, substance use disorder, and/or foster care to assist in supporting youth in their resiliency/recovery and wellness
n
nnMust have, or are willing to complete the required state training for their Provisional Peer Credential
n
