Job description
Summary:
Responsible for assessing patient needs and linking patients to community agencies and resources as dictated by patient need.
* Conducts an initial assessment and gathers data relevant to patient needs.
* Assess patient needs and capabilities in the context of specific referral requests made by the primary clinician.
* Gathers and documents all necessary information needed for making patient referral, e. g. , proof of patient income, current medication dosages, etc.
* Coordinates services and resources to meet patients ancillary needs.
Experience:
* 1-3 years
Credentials:
* Bachelor's Degree in Human Services or related field required.
* Valid MD Driver's License required and clearance by the LBH Fleet Safety Department to operate vehicles.
Pay Range: $20.06 to $30.09 Based On Experience
